NOPR Information for Physicians
| For a patient to participate in the Registry, the patients referring physician must complete a brief questionnaire before and after the PET exam. For your convenience, the Case Registration and Pre-PET Form is attached below, along with the Patient Informational Sheet you will need to provide the patient. After the PET exam is completed, Central Scheduling will send you the patient-specific Post-PET Form to complete. |
