Congestive heart failure (CHF) is a serious form of heart disease, but it can be monitored and treated to improve heart function and reduce symptoms. The Forsyth Heart Failure Program includes an outpatient clinic that provides comprehensive care to people with all stages of heart failure. The program aims to help patients improve their quality of life and decrease their hospitalizations.

Our cardiologists, nurse practitioners, and case managers have extensive experience and training in the management of patients with heart failure and coordinate their recommendations with your physician.
