We understand your concern about sharing your social security number and will do our utmost in protecting this information. We are required by federal law and Forsyth Medical Center policy to protect the information you share with us. Employees will be held accountable up to termination for any intentional breach of your privacy.
Why does Forsyth Medical Center request my Social Security Number?
We ask for your social security number in order to bill your insurance company. A majority of health insurance companies require a social security number in order to process the payment of billing claims. Typically, health insurance companies use your social security number for identification purposes.
Do I have to give Forsyth Medical Center my Social Security Number?
No, you are not required to provide your social security number to Forsyth Medical Center. Giving your number is voluntary, even when you are asked for the number.
What may happen if I refuse to give my Social Security Number?
If your insurance company uses your social security number as identification, refusing to provide the number means that we will be unable to bill your insurance company. If we are unable to bill the insurance company, Forsyth Medical Center will require payment in full when services are provided.
I thought only the government could use my Social Security Number?
The Social Security Number was originally created to keep records of your earnings and to monitor benefits paid. However, the social security number has become the most widely used form of identification for record keeping throughout all types of businesses. The government does not restrict the use of it for these purposes.
How can I be sure that Forsyth Medical Center protects my Social Security Number?
We are required by Federal Law to protect the information you share with us. Forsyth Medical Center policies are in place to protect the confidentiality of all patient information. Staff is required to sign a confidentiality agreement. Any breach in confidentiality by a Forsyth Medical Center employee will result in disciplinary action up to termination of that employee.
Must I provide a Social Security Number when being treated in the Emergency Department?
No. Forsyth Medical Center will provide medical screening and stabilizing treatment regardless of what (if any) information patients are willing or able to provide. Once stabilizing treatment has been provided, patients who elect not to provide their social security number will need to make payment arrangements if their insurance company requires this information to process an insurance claim.
