Every patient at Forsyth Rehab Center receives a customized plan of treatment after evaluation. In addition to rehabilitation nurses and occupational therapists, physical therapists and speech language pathologists; the staff includes physiatrists (physicians trained in rehab medicine), psychologists, social workers, registered dieticians and others. Patients also benefit from a Care Coordinator who meets regularly with the patient and family to review progress towards goals, coordinates the activities of the treatment team and provides an opportunity for the patient/family to give input into goal setting.

Bedside fiberoptic endoscopic evaluations of swallowing

Cooking groups to evaluate safety and function in the kitchen and teach skills to improve independence in the home setting

A wide range of speech language pathology treatments

Activities of Daily Living apartment used for overnight or weekend stays by patients and families. This provides a home-like setting to prepare for return to home.

North Carolina Assistive Technology Project Office, located on-site, provides a wide range of assistive devices to try with patients

A community re-entry program directed by Recreation Therapists provides opportunities for patients to take short trips into the community. These outings may include shopping, banking or dining out to improve their safety, confidence and socialization skills prior to discharge
