And for those patients who are well enough to leave the hospital but are still facing serious cognitive or physical challenges, extra attention from specially trained stroke care staff can make a big difference. At Forsyth Medical Center's Stroke Bridge Clinic, patients can get this extra attention in a friendly practice setting that helps them transition from the hospital to home.
At the Stroke Bridge Clinic, a neurology nurse practitioner reviews patients' treatment plans to make sure that recovery is progressing as it should. A stroke navigator also guides them and their caregivers through the recovery process, answering questions, helping with arrangements such as transportation to appointments, and helping them connect with services in the community that can make life easier as they recover. Sometimes, the navigator just provides a sympathetic ear and words of encouragement on days when recovery seems the most challenging.
The Stroke Bridge Clinic staff also works with a patient's regular physician to ensure that they make a smooth transition back to primary care.
