Stroke Navigator

A stroke navigator is a member of your healthcare team who is here to help you understand more about your diagnosis, treatments, what will happen while you are a patient here and assist you on the road to recovery after discharge from the hospital. These services are free and may include:

• Help in communicating with your healthcare team including doctors, nurses, therapists, dieticians, case managers and other healthcare professionals
• Guidance and communication about your medical diagnosis and treatment options
• Help with identifying hospital-based and community resources for you and your family
• Assistance finding information to help you and your family better understand your diagnosis
• Offering emotional support
