It is important to know and understand your heart health. This free assessment tool will help you learn about your heart disease risk factors and will suggest practical steps to take to improve your health.
Before you start, you may want to have the following information available. This will help to make sure that your heart risk assessment will be as complete and accurate as possible. If you do not have this information available, you can still do the assessment.
- Height
- Weight
- Waist Circumference
- Cholesterol - Total, LDL, HDL, Triglycerides
- Blood Glucose (Sugar)
Once you begin, you can stop and continue at a later time. If you plan to complete the assessment in one sitting, schedule about one hour.
At the end, you will receive a link to your risk assessment report and links to Web pages with information about classes and services that can help you reach your goals.
